Admission Criteria
· Exhausted insurance coverage/benefits for therapy services
· No available funds to cover cost of therapy
· Prescription from physician licensed in the state of Oklahoma
· Unresolved impairments and/or functional limitations
Continuation of Treatment
· Frequency and Duration of treatment will be individualized and addressed on a case-by-case basis
· Referrals/recommendations for additional services will be made as deemed appropriate and may preclude participation in the free clinic
· Effectiveness of outcome will be assessed on an ongoing basis for program continuation; however, a maximum limit of six-month duration will be the norm for each individual patient.
· Willingness of patient to work and consent for treatment (and waiver of liability)
· Active participation by family/support members with follow-through/consistent practice of recommended exercises/activities.
Documentation
Each participant will have a medical chart with a record number. Charts will include patient information sheet (biographical data, diagnosis, history, goals, etc.).
